Chronic Obstructive Pulmonary Disease Quality Standard
This Implementation Toolkit is a comprehensive guide to improving care for those living with chronic obstructive pulmonary disease (COPD). This implementation toolkit is created for clinicians, health system planners, and health organizations to help them implement Ontario Health’s Chronic Obstructive Pulmonary Disease (COPD) Quality Standard, and consists of resources, point-of-care tools, videos, portals, virtual programs, digital platforms, and more.
By integrating evidence-based provincial quality standards and clinician-vetted resources and tools, this toolkit supports the implementation of high-quality, evidence-based COPD care across the disease spectrum from early diagnosis to end-of-life care.
Ontario Health’s COPD Quality Standard
Ontario Health’s quality standard Chronic Obstructive Pulmonary Disease: Care in the Community for Adults (the COPD quality standard) addresses care for people living with COPD, including the assessment of people who may have COPD. It provides guidance on the diagnosis, management, and treatment of COPD, and it applies to primary care, specialist care, long-term care, and other home and community care settings. This quality standard does not address care provided in an emergency department or hospital inpatient setting for the management of acute exacerbations of COPD.
The COPD quality standard consists of 14 quality statements that have been summarized into a pathway to help those implementing the standard envision an ideal future-state model of care for people living with COPD.
